FORM FOR THE FOLLOWING
HAWAII VACATION:
This certificate is
redeemable for a
reservation form,
offering two adults ages
21 or older to 5 days
and 4 nights at the
destination of choice in
Hawaii. At the time of
booking your
reservations, a US $139
per person reservation
and processing fee,
double occupancy, is
required and is payable
to the fulfilling travel
agency that will book
your reservations.
Additional nights are
available at $99 per
night. Additional
travelers require a $35
per person surcharge.
Resort accommodations
are available year
round, subject to
availability and a
minimum 30-day advance
notice. Reservation
requests made less than
30 days in advance of
arrival are accepted
based on availability
and subject to a $49
surcharge.
In most cases
you will be required to
give the travel agency
multiple travel dates
just in case your 1st
requested travel dates
are not available. Group
travel requires a $49
per person surcharge.
Transportation, taxes,
gratuities, meals, and
other expenses are not
included with this
offer.
